Applications can be submitted not more than 90 days prior of entering the Schengen territory

We will not process incomplete applications and we can reserve the right to reject your visa request. If the items listed below are not presented, the applicant will have to reschedule a new appointment.

Visa Application Form.

Recent passport-size photograph on white background, full face and front view, stapled/glued to the application form. Scanned or photocopied photos will NOT be accepted;

Valid passport (plus 1 copy of the main page). The passport or travel document must be valid at least three months after the visa expiration date. Please make sure the passport is no older than 10 years and has at least two blank pages to affix the visa;

Original Permanent Residence Card (plus 1 copy front and back side) OR other US immigration document allowing re-entry into the United States (e.g., valid long-term US visa, Advanced Parole, etc.). International students must submit original of F-1 Visa and I-20 ( plus 1 copy of each document)

Original of Driver’s License or State ID (plus 1 copy) as proof of residence in the jurisdiction of this Consulate General. If you recently moved and have not updated your ID, you must submit another proof of current address (e.g., utility bills, bank statements, etc.);

Copies of most recent Schengen Visas ( if applicable), copies of entry/exit stamps from Schengen Border Authorities.

Copies of all the visas needed for the current trip;

Letter from the US employer on company letterhead specifying:

name of the applicant;

employment status and purpose of the trip;

complete period of travel;

full financial responsibility for expenses and lodging by the US Company;

Self-employed or owner of a company:

copies of corporation papers

most recent tax return

Letter of invitation from the Italian company on original letterhead specifying:

name of the applicant;

purpose of the trip;

intended period of stay in Italy.

copy of the "visura camerale" (certificate of deed)

copy of the Italian ID of the person signing the letter of invitation.

Conference Registration: those going to attend a conference in Italy are required to provide a copy of the Conference registration with location and date of the Conference.

Proof of lodging for the entire duration of the trip and for all the countries you intend to visit:

Confirmed hotel reservations with name and address of hotels

Medical Coverage Abroad for the entire period of travel in the Schengen area with a minimum coverage of $50,000 for hospitalization, emergencies, evacuation and repatriation.

Flight reservation for a round trip showing complete itinerary. If you visit more than one Schengen country, please provide means of transportation between countries;

Visa fee (non refundable). This Consulate accept only Money Orders, Cashier's checks or cash payable to the Consulate General of Italy, We do not accept personal checks or credit cards;

Mailing Applicants can provide a SELF-ADDRESSED, PRE-PAID EXPRESS MAIL ENVELOPE to receive the passport and visa by mail. Envelopes with insufficient postage will not be accepted. Passport and visas can be always collected at this Office during regular business hours. WE DO NOT ACCEPT REGULAR MAIL.

First-time applicants have to appear in person to place a visa application in order to provide their photograph and fingerprints – biometrics. For subsequent applications within next 5 years, the biometric data will be retrieved from the VIS SYSTEM and applicants can apply by mail. After 5 years, the biometric data will be deleted and a new in person application will be required.
