In order to register in Italy a death occurred in the territorial jurisdiction of this Consulate General - Arkansas, Louisiana, Oklahoma and Texas -, the following documents must be provided by mail to this Office:
1. Certified Copy of the Death Certificate in LONG FORM, issued by the competent Vital Statics Office (see below), duly legalized with Apostille (the Apostille is issued by the competent Secretary of State – click here).
2. Translation of the document into Italian language - see list of translators; Apostille does not need translation.
3. Form to request the registration of the Death Certificate duly filled out and signed - click here.
4. Copy of the applicant's valid passport.
