We suggest emailing the Ufficio Anagrafe\/Stato Civile of your Comune (write in Italian)<\/li>\n<li>With the local authorities <strong>at the time of the birth registration<\/strong>, make sure the names, DOB, POB of the parents are <strong>correct<\/strong> and <strong>exactly match<\/strong> the Italian documents. In case of discrepancy, make sure the final document is correct<\/li>\n<li>The Surrogate motherhood is a crime [<a href=\"https:\/\/conshouston.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/stato-civile\/nascita-derivata-da-maternita-surrogata\/\" target=\"_blank\" rel=\"noopener\">Birth derived from \u201csurrogate motherhood\u201d \u2013 Consolato Generale d&#8217;Italia Houston<\/a>]<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Documentation to present to this Consulate<\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li><u>If the minor was born <strong>after<\/strong> the marriages of the parents and the <strong>marriage has not been registered in Italy yet<\/strong><\/u>, is necessary to present also the documentation to register the marriage. Previous marriages and divorces\/deaths must be registered as well.<\/li>\n<li><u>If the parents <strong>are not married or were not married at the time of the birth<\/strong><\/u>, is necessary to present the \u201cAcknowledgment of Paternity\u201d or \u201cCertificate of Parentage\u201d with Apostille\/Legalization and Translation.<\/li>\n<li><a href=\"https:\/\/conshouston.esteri.it\/wp-content\/uploads\/2023\/07\/Form-Nascita.pdf\">Birth Application form<\/a>, completely filled out except for the date and signature<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><span style=\"color: #ff0000;\"><strong>FOR DOCUMENTS ISSUED IN TEXAS, LOUISIANA, OKLAHOMA, ARKANSAS<\/strong><\/span><\/p>\n<ul style=\"text-align: justify;\">\n<li><strong>Original Birth Certificate<\/strong> (o Certified Copy issued by the competent County Clerk). The certificates must be issue in <strong>Extended Form<\/strong> (or <em>Long Form<\/em> showing the exact place of birth, date\/age and DOB for both parents) according to the State procedures<\/li>\n<li><strong>Apostille<\/strong> from the Secretary of State of the state that issued the certificate.<\/li>\n<li><strong>Translation <\/strong>in Italian of the certificate: typed, exactly matching the original document, with dates in the format dd\/mm\/yyyy and hours in 24h format. List of suggested translators at <a href=\"https:\/\/conshouston.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-straniero\/apostille-translation-and-legalisation-of-documents\/translators\/\" target=\"_blank\" rel=\"noopener\">Translators \u2013 Consolato Generale d&#8217;Italia Houston<\/a>.<\/li>\n<li><strong>Copy of the valid passports of both parents <\/strong>(only the page with picture, data, signature)<\/li>\n<li><strong>Copy of the minor\u2019s passport<\/strong>, if possible.<\/li>\n<li>If parents were not married at the time of birth, is necessary to present also:<\/li>\n<\/ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul style=\"text-align: justify;\">\n<li>The \u201c<strong>Acknowledgment of Paternity<\/strong>\u201d or \u201c<strong>Certificate of Parentage<\/strong>\u201d. It is usually completed and signed by unmarried parents at the hospital and subsequently registered together with the birth at the local vital records office, it can therefore be obtained from the same offices that issue birth certificates<\/li>\n<li><strong>Apostille<\/strong> on the &#8220;Acknowledgment of Paternity\/Certificate of Parentage\u201d<\/li>\n<li><strong>Translation <\/strong>in Italian of the certificate: typed, exactly matching the original document, with dates in the format dd\/mm\/yyyy and hours in 24h format.
