Passports for adults

On the day of the appointment, show up with:

1. the form for adults, completed but not signed;
2. the previous Italian passport, if you hold one (*);
3. local driver’s license (or local ID), if you hold one (*);
4. passports of other nationalities, if in possession of multiple citizenships (*);
5. US legal residence permit (US passport, US Permanent Resident Card, US Employment Authorization Card, or US Visa), if you hold one;
6. certificate of US naturalization, if naturalized;
7. 2 passport-size color photographs 2″x 2″, front view, clear background, unsmiling; the size of the face must not exceed 1.18″ in width and 1.38″ in height;
8. Money Order (only USPS) or Cashier Check (Pay to “Consulate General of Italy – Houston” From “SURNAME Name” Memo “SURNAME Name / Passport) for a total of $125.00. Check that the amount is the one indicated in the updated Estratto tabella consolare III trimestre 2024. Please be aware that the amount change on Jan 1^st , April 1^st , July 1^st  and October 1^st ;
9. A USPS or UPS pre-stamped and pre-addressed envelope with tracking number (no FedEx). Only one envelope can be brought for multiple bookings. In case of pickup at the Consulate, the envelope is not required;
10. The shipment authorization form (LINK), completed and signed;
11. In the presence of minor children, we point out that following the approval of the D.L. 69 of 13 June 2023, the consent of the other parent will NO longer be necessary for the issuance of an individual passport. The assent will be replaced by a declaration in lieu of certification (now included in the form for adults);
12. Photocopies of all documents submitted in 2,3,4,5,6

(please note that at least one document marked with the symbol (*) must be valid and complete with photo and signature)
